20251024 Quality Alliance Lunch and Learn: Managing a Busy PCP Practice with 65+ Patients

Physicians, Advanced Practice Providers, Residents, and Nurses.
Learning Objectives
Upon completion of this activity, the participant should be able to
- Identify opportunities to decrease emergency room (ER) utilization by leveraging the care team, refining when an ER referral is needed, and improving access to resources
- Incorporate advance care planning (ACP), sociological determinants of health (SDOH) assessments, medical power of attorney (MPOA), and out of hospital do not resuscitate (OOH-DNR) into workflows and understand how comprehensive care management (CCM) can support these efforts
- Address risk adjusted factor/hierarchical condition categories (RAF/HCC) to capture an accurate risk adjustment for geriatric patients
