20251205 Quality Alliance 65+ Lunch and Learn Series: Falls and Frailty
Target Audience
Physicians, Advanced Practice Providers, Residents, and Nurses.
Learning Objectives
Upon completion of this activity, the participant should be able to
- Define the clinical syndrome of frailty and its phenotypic components.
- Demonstrate how to use validated tools for assessing frailty and fall risk (e.g., Clinical Frailty Scale, Palliative Performance Scale, Timed Up and Go (TUG), Tinetti, STEADI algorithm, Sit to Stand Test.)
- Analyze evidence-based interventions for preventing and mitigating frailty and falls, including exercise programs, nutritional strategies, medication optimization, and environmental modifications.
- Apply interdisciplinary care strategies for frail older adults including coordination with physical and occupational therapy, care management for patient education, care coordination and supportive palliative care.
