Primary care providers know there is a growing need to address how we care for our senior population. This symposium will address the knowledge gaps about what tools are available and how to utilize. Primary care physicians must be equipped with strategies to appropriately care for our senior population.

Learning Objectives
Upon Completion of this activity, the attendee should be able to:
- Increase pre-palliative care/pain control for seniors
- Incorporate advance care planning, out of hospital-do not resuscitate (OOH-DNR), medical power of attorney (MPOA), legal, housing into workflows
- Address risk adjusted factor/hierarchical condition categories (RAF/HCC) for senior and how to risk adjust your senior
- Identify how to decrease utilization by increasing primary care physician (PCP) utilization/care team
