20260130 Quality Alliance Lunch and Learn: 65+ Population: Diabetes Management
Target Audience
Physicians, Advanced Practice Providers, Nurses, and Pharmacists.
Learning Objectives
Upon completion of this activity, the participant should be able to:
- Apply individualized glycemic targets and de-intensification strategies for adults ≥65 based on comorbidity, cognition, and functional status.
- Evaluate and adjust diabetes medication regimens by assessing risk–benefit profiles, renal function, and hypoglycemia risk to ensure safer prescribing for older adults.
- Describe the unique challenges faced by the 65+ population in managing diabetes, including clinical, social, nutrition, cognitive, and functional factors
- Interpret and act on CGM and patient-generated health data (e.g., time-in-range, hypoglycemia exposure) to guide treatment decisions for older adults.
- Integrate strategies to address clinical, nutritional, cognitive, and social challenges—including SDOH barriers—into individualized care plans for older adults with diabetes.
