OBJECTIVES: Upon completion of this activity, the participant should be able to: 

1. Prepare practical guidance for surgeons to use on a daily basis in order to facilitate goal-oriented care planning.
2. Plan to educate on available resources in our system intranet, as well as EMR, for purposes of efficiency.
3. Discuss the importance of utilizing available education tools and advanced care planning documents to meet our goals of providing care focused on quality, safety, and alignment.
