OBJECTIVES: Upon completion of this activity, the participant should be able to:
- Explain normal insulin physiology
- Recognize how different insulin formulations can be used to correct abnormal glucose levels
- Discuss how to use patient history, HgbA1c, weight, and initial glucose levels to estimate initial inpatient insulin treatment
- Formulate initial inpatient insulin treatment with physiological balance of basal, nutritional, and correction scale insulin
- Determine correction insulin scale based on total daily insulin dose (TDD)
- Apply percentage changes in insulin doses to refine management
- Discuss how to adjust insulin treatment for steroid therapy and chronic kidney disease
- Determine discharge regime for glucose management based on historic outpatient control, inpatient insulin response, and patient socioeconomic considerations
