OBJECTIVES:

  • Identify type of error, i.e. individual or system and significance of error, i.e. major or minor and determine its role in the patient outcome.
  • Define expert consensus and literature review to implement process and/or quality improvement and develop a pattern of self-reflection and system accountability in the care of critically ill patients.
