OBJECTIVES: Upon completion of this activity, the participant should be able to:
1. Define the current racial/ethnic health care disparities in the United States. Outline the relationship between racial/ethnic health care disparities and hospital admissions.
2. Review the Center of Medicare and Medicaid Services Hospital Readmission Reduction Program and the proposed strategies to prevent hospital readmissions. Divide into the literature to explore the specific racial/ethnic health care disparities that have uncovered by the CMS Hospital Readmission Reduction Program.
3. Discuss the potential strategies in which hospitalists can use to address the racial/ethnic health care disparities but at the same time prevent hospital readmissions.
