OBJECTIVES:  Upon completion of this activity, the participant should be able to: 

1)  Recall the history of social determinants of health and its impact on providing care to our patients.
2)  Discuss how we are bridging the gap between inpatient and outpatient to address social determinants of health of patient population.
3)  Recognize the social determinants of health data that has been collected by Baylor Scott and White to date.
4)  Underline the future implications of addressing and assessing the social determinants of health of our patient population.
