OBJECTIVES: Upon completion of this activity, the participant should be able to: (1) Define the three types of microtrdansitions and identify the risks they pose to the physical and psychological health of post-acute and long-term care patients. (2) Explain how clinicians of all specialties can foster safe and effective microtransitions for patients. (3) Apply the best practice recommendations for facilitation of effective microtransitions.
