OBJECTIVES: Upon completion of this activity, the participant should be able to: (1) Describe why structured debriefing after challenging or unanticipated clinical outcomes is important for patient safety, team learning, and clinician well-being. (2) Identify key components of an effective debrief, including psychological safety, clarity of purpose, and a focus on systems rather than individual blame, (3) Develop strategies for "closing the loop" by translating issues raised in debriefs into concrete follow-up actions (i.e. process changes, communication with leadership, or safety event reporting)
