OBJECTIVES: Upon completion of this activity, the participant should be able to:
1.Describe the development, immunological, and sensory-neural pathophysiology that drives extreme perioperative airway hyperreactivity in infants and young children, including the prolonged effects of recent viral respiratory infection.
2.Summarize contemporary closed-claimed and registry data demonstrating that perioperative respiratory adverse events in children with reactive airways remain the leading source of malpractice liability, permanent injury, and mortality in pediatric anesthesia.
3.Analyze stratify perioperative risk for airway hyperreactivity using validated clinical tools, recent registry findings, and pathophysiology insights applicable in both high-volume and occasional pediatric practice.
4.Formulate and execute evidence-based induction techniques supported by 2003-2025 trials and guidelines that substantially reduce the incidence and severity of perioperative respiratory adverse events.
5.Recognize and manage acute intraoperative reactive airways events via a structured, mechanism-directed algorithm informed by current literature, achieving rapid restoration of oxygenation and airway patency.
6.Design and implement evidence to minimize and manage PACU pediatric respiratory events.
