Overview: Physician documentation is translated into codes. Codes are used to calculate numerous metrics. Your documentation in stroke patients has important impacts on these metrics!  
Patients often are admitted for “Stroke-like symptoms”.  It is important to confirm the stroke diagnosis in your documentation   once diagnostic criteria are met.
