Providers need to better strategies to understand the documentation that is required for specific code assignment so that the medical diagnosis codes are an accurate reflection of their patient’s condition, severity, complications/manifestations, and treatment.

Learning Objectives

Upon completion of this activity, the participant should be able to:

Review how documentation reflects the complexity of patients and affects risk adjustment.
Identify the key components of documentation that are required for accurate code assignment for Cardiac related HCCs such as Heart Failure, AMI, Arrhythmias.
Implement best practice recommendations for documentation to maximize Cardiac HCC coding
