After participating in this activity, the learner should be able to:

  1. Review how documentation reflects the complexity of patients and affects risk adjustment.  
  2. Identify the key components of documentation that are required for accurate code assignment for Cardiac related HCCs such as Heart Failure, AMI, Arrhythmias.  
  3. Implement best practice recommendations for documentation to maximize Cardiac HCC coding
