Overview:
At Baylor Scott & White Health (BSWH), Medicare Advantage patients make up approximately 40% of revenue at a $640 million loss, relative to the cost to deliver service. Seniors are the fastest growing segment of the population with approximately 480,000 patients aged 65 years and older matriculating through the system each year. To achieve long term financial sustainability, BSWH must develop an effective senior strategy to serve this segment. This is why the 360 Visit was created. The 360 Visit is a dedicated, intentional visit to capture Hierarchical Condition Category’s (HCC), close care gaps, and align resources to customer needs. In doing so, this will align funds with appropriate risk assessment to provide resources for patient care.

Learning Objectives
After participating in this activity, the learner should be able to:
- Participants will be able to identify and schedule patients who are candidates for a 360 Visit.
- The healthcare provider will utilize Curation to capture HCCs.
- The learner will be able to perform an appropriate referral to Comprehensive Care Management (CCM).
- The healthcare provider will complete the Medicare Wellness Visit template in Epic during the visit to address social determinants of health.
