Learning Objectives

- Identify and implement strategies to potentially lessen the 27% prevalence gap BSWH has compared to CMS national average for incidence of coding ischemic heart disease codes
- Apply effective documentation practices to ensure all relevant ischemic heart condition codes are captured
- Utilize actual BSWH provider documentation examples to show opportunities for improvement in the documentation and subsequent coding of ischemic heart disease conditions
