Baylor Scott and White has identified heart failure as a major contributor to emergency department visits. The Baylor Scott & White (BSW) Health Employee Plan and Medicare Advantage Plan populations are both significant emergency department utilizers for heart failure.

Target Audience

All providers caring for the heart failure patient.

Learning Objectives

Upon completion of this activity, the participant should be able to:

  • identify and prescribe the most appropriate treatment for HFrEF.
  • perform an appropriate specialist referral for patients with a HFrEF.
  • consider cost tolerability through use of the Medication Access Tip Sheet when treating patients with HFrEF.
