In risk adjustment, International Classification of Diseases (ICD) diagnosis codes serve as one of the main building blocks for calculating the risk adjustment factor score. In many instances, the more accurate and detailed the diagnosis coding, the higher the risk score — assuming the condition is designated as a Hierarchical Condition Category (HCC) diagnosis and that the diagnosis is clinically appropriate. Accurate code assignment is vital in the risk adjustment process as it directly influences the risk score, which determines reimbursement rates for healthcare providers and payers. With improved documentation practices and knowledge of risk adjustment, providers can contribute to improved healthcare management and ensure that resources are allocated appropriately to meet the needs of patients.
