OBJECTIVES:   Upon completion of this activity, the participant should be able to:

  • Incorporate stroke survivor perspectives into patient‑centered communication strategies to more effectively address long‑term recovery challenges.
  • Apply survivor‑informed approaches to care planning and shared decision‑making across the post‑acute and outpatient stroke care continuum.
  • Identify actionable opportunities to modify follow‑up, referral, and interdisciplinary care strategies based on survivor‑reported needs and barriers.
