Special author instructions for CME articles

Each issue, readers of the Proceedings can receive CME credit for carefully reviewing and interacting with certain articles that are published in our journal. We have chosen your article as one of the CME offerings for the coming issue, and we need extra information from you to complete this process.

____ 1. Faculty disclosure. Please have each author click here to complete the disclosure form.

____ 2. Learning objectives. Please attach a list of the major learning objectives for your article (usually 1-3 objectives per article).

Objectives should be written in terms of what the participant should be able to do after completing the activity:

Example: “After completing the article, the learner should be able to apply the principles of safety and asepsis in the performance of all aspects of patient care regardless of the clinical environment in with the practice resides.”

____ 3. Faculty credentials. Please provide a sentence on each author’s credentials.

____ 4. Quiz questions. Please submit 2 to 3 multiple-choice questions and answers for CME participants. Ideally, each question should be a case-based question reflective of your content. The rationale for the correct answer along with a citation for further understanding is required for each question.

____ 5. Further reading. If you wish to refer participants to materials other than those cited in your references, please provide full citations on a separate sheet of paper.
