Health is more than just the physical absence of disease.  Health is also affected Health is also determined by social and economic opportunities, the lack or absence of resources and support systems in our homes , neighborhoods and communities, safety in our homes and communities, access to water, food and housing and finally the nature of our social interactions and relationships.  The conditions and environments in which we live help explain why some individuals are healthier than others and why some environments and conditions create a sense of stress and a feeling of being unsafe.  All of this may lead to a state of unhealthiness.

Create a network among healthcare providers and community agencies and resources to better align and decrease and minimize the influence of social factors on overall health of our communities.

Provide accurate and up to date information regarding social issues which may affect a sense of wellbeing of anyone.
