Medicine Grand Rounds is designed to review existing cases and apply the most current evidence-based knowledge and guidelines to establish treatment plans for improved patient outcomes.

The purpose of the Internal Medicine Grand Rounds series is to provide the Internal Medicine physician audience with lectures to allow physicians to improve techniques involved in the diagnosis and management of disease often seen by Internal Medicine Physicians.

OBJECTIVES:  Upon completion of this activity, the participant should be able to:  (1) Recognize advanced heart-failure as a spectral, time-sensitive entity associated with high mortality, (2) Value the importance of early referral to plan a transition strategy focused on three pathways native heart survival, replacement and/or palliation, (3) Review contemporary deep phenoprofiling of advanced heart disease strategies using clinical, hemodynamic, multimodality imaging to create a transition plan in advanced heart failure.
