This lecture will address how assessment, care, multi-disciplinary team referrals, and coding can efficiently and effectively improve related management outcomes long-term.

  • Examine patient and complete risk assessment.

  • Recognize signs and symptoms before escalating, if needed.

  • Assess scope and appropriately refer to necessary specialists.

  • Apply best practice recommendations for diabetes documentation to maximize HCC coding.
