Learning Objectives
Upon completion of this activity, the participant should be able to:
- Discuss the prevalence and significance of polypharmacy in older adults.
- Select best practice strategies to reduce polypharmacy and promote appropriate prescribing and deprescribing.
- Describe the Centers for Medicare and Medicaid Services (CMS) polypharmacy quality measures in effect for the 2025 measurement year.
- Complete a medication review with the goal of reducing polypharmacy and identifying deprescribing opportunities.
